New Patient Appointments

We thank you for scheduling your new patient appointment with Eastpointe Internists. To make your encounter as favorable as possible, we ask that you follow these simple instructions.

  • Please print off the registration form and confidential health history form, which are available here.
  • Take a few minutes to fill out and bring them with you to your appointment.
  • We ask that you arrive 15 minutes before your scheduled appointment so that the paperwork can be completed in a timely fashion.
  • Bring any insurance cards to your office visit, as we will make a copy for our files. A friendly reminder - you are responsible for any co-payments, deductibles or noncovered benefits at the time of your service.
  • Make a list of the questions and points you want to discuss with your provider.
  • Bring your current medication bottles to your first appointment. It is sometimes difficult to remember all your current medications.
